Administer patient assessments and oversee the assessment process; set assessment schedules and assure assessments are done accurately and timely. Coordinate the care plan according to regulatory requirements. Create schedules for all Medicare and Medicaid; initiate Medicare coverage for newly qualified patients or issue denial letters; stay updated on changes in Medicare coverage and determine documents needed for Medicaid reimbursement. Direct the day-to-day functions of the nursing aides in accordance with current rules, regulations, and guidelines that govern the long term care facility. Ensure all nursing personnel comply with the facility’s written policies and procedures; periodically review the department’s policies, procedure manuals, job descriptions, and recommend revisions. Plan shifts, services, programs, and activities with assigned nursing staff and support personnel; ensure the Nursing Service Procedures Manual is current and reflective of day-to-day nursing procedures; ensure compliance with the manual’s procedures. Make written and oral reports/recommendations concerning shift activities; cooperate with other resident services to maintain the resident’s total regimen of care; participate in the facility’s quality assurance program for the nursing service department. Participate in facility surveys (inspections) conducted by authorized government agencies as requested; periodically review and update the resident’s discharge plan as required; assist in planning the nursing services portion of the resident’s discharge plan. Interpret department policies and procedures to personnel, residents, visitors, and government agencies as required; admit, transfer, and discharge residents as required; complete accident/incident reports as necessary. Coordinate charting and documentation: complete required recordkeeping forms/charts at admission, transfer, and discharge; encourage physicians to review and sign orders and progress notes; transcribe orders to resident charts; document nursing notes descriptively; complete accident/incident reports and forward to the Director; ensure discharge and diet information is properly communicated and documented. Drug administration: prepare and administer medications as ordered; verify resident identity; maintain narcotic records; manage drug/discrepancies and disposal per procedures; communicate automatic stop orders to attending physicians as required. Personnel functions: participate in evaluations, determine staffing requirements, and make recommendations regarding personnel actions; manage shift staffing and performance; supervise nursing personnel and ensure adherence to policies; conduct regular rounds; ensure staff are graduates of approved CNA programs or under licensed supervision. Enforce policies for residents, staff, visitors; maintain interdepartmental relationships to support resident care; create a warm, positive, and calm unit environment; review complaints and grievances and report to the Nurse Supervisor; follow facility procedures. Information security and privacy: safeguard user IDs/passwords and protect residents’ protected health information; report unauthorized access attempts and follow procedures for information security. Nursing care functions: inform staff of admissions and room assignments; participate in orientation for new admissions; coordinate diagnostics/therapies; consult with physicians; review care plans and ensure adherence to treatments; supervise CNA performance and ensure alignment with the care plan; implement restorative/rehabilitative programs; supervise infection control and safety practices; administer procedures, including use of restraints as required by policy; collect specimens and document vitals; monitor seriously ill residents; coordinate meal-related changes and ensure dietary allowances are followed; coordinate post-discharge transportation and family communications; ensure post-mortem procedures when applicable. Staff development: develop and deliver in-service training; orient new employees to shift policies; standardize work methods; train staff on blood/body fluid exposure; support clinical supervision for nurse aide trainees; participate in outside training and ongoing education programs (e.g., OSHA, HIPAA, Safety, Infection Control). Safety and sanitation: monitor safety adherence; ensure PPE availability; maintain clean and sanitary work areas and patient rooms; enforce hand hygiene; participate in fire safety and disaster drills; contribute to infection control program; follow isolation precautions when necessary; ensure safe operation of equipment; document and report hazardous conditions; encourage proper labeling and use of MSDSs. Equipment and supply: recommend equipment and supply needs; maintain adequate stock levels; ensure safe operation of nursing service equipment; verify that only trained personnel operate equipment; promote efficient use of supplies. Care plan and assessment: review care plans daily and communicate changes to the Nurse Supervisor; ensure CNAs refer to the care plan; assist in planning, scheduling, and revising the MDS and RAPs/Triggers; verify that CNAs follow the care plan when delivering daily care. Budget and planning: report suspected fraud related to billing, cost reports, or kickbacks as applicable; provide data to the Quality Assurance & Assessment Committee as requested. Qualifications

Education and/or Experience: Must have a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program; current, unencumbered active license to practice as an LPN/LVN in the state. Language: English – must be able to read, write, speak, and understand; ability to read technical procedures and policy manuals; effective communication with managers and employees. Mathematical and Reasoning: Ability to apply fractions, percentages, ratios, and proportions; provide age-appropriate care; leadership and supervisory ability; independent decision-making when warranted; tactful interaction with residents, families, staff, and agencies; knowledge of nursing practices, regulations, and guidelines; ability to plan and implement programs and maintain a collaborative team environment; patience and adaptability; willingness to adopt new methods. Certificates, Licenses, Registrations: Current CPR certification; PCC experience preferred; maintain continuing education/licensing; be in good standing with the State Board of Nursing. Physical Demands: Mobility throughout the day; ability to communicate in English; manage mental and emotional stress; vision and hearing capabilities; ability to lift and move at least 25 pounds, with flexibility for longer distances; may assist in evacuations. Work Environment: Office and clinical settings; frequent interruptions; exposure to residents, staff, and visitors; possible after-hours work and emergency call-backs; participation in continuing education; exposure to potential hazards (bodily fluids, chemicals, waste) and infectious agents; willingness to coordinate with others to meet resident needs.
