Reporting directly to the ACO REACH Program Manager, the

Program Specialist

will be a pivotal contributor to the operational excellence and strategic growth of our ACO REACH initiatives. This role is responsible for the intricate management and refinement of core program processes, ensuring seamless execution across all facets of ACO REACH. The Specialist will analyze program performance, identify opportunities for optimization, and collaborate with diverse internal teams to implement solutions that enhance engagement and outcomes within the ACO REACH framework. This position translates strategic vision into operational reality, driving continuous improvement and scaling of the ACO REACH program.

Primary Duties

Working and supporting day-to-day ACO REACH processes and operations

Manage Zendesk incoming/outgoing communications for the ACO REACH team, handling all zendesk tickets and escalating as needed

Build out solutions in Monday.com for ACO REACH process and operations needs (Growth, SVA, Contacts, Practice lists, Provider lists, CMS tickets, etc.)

Utilize Medicare reports from 4i (quality, financial, claims reduction files, SVA response files) to generate reports and identify actionable insights

Use reports, calendars and internal tools (Tableau, Snowflake) to build internal and external decks for Contract and Resource Meetings, and Performance and Resource Meetings

Draft and maintain Aledade ACO REACH newsletter and manage distribution

Manage the Beneficiary Zoom Voicemail for the ACO REACH team, ensuring questions are answered and logged in Monday.com

Serve as a trusted planner, organizer, and executor for the ACO REACH team

Plan in Monday.com based on the ACO REACH calendar distributed by CMS to schedule deadlines, internally and externally

Maintain ACO REACH Google Drive folders with appropriate organization and access

Provide thoughts on opportunities for process development, needs or gaps

Offer suggestions and recommendations for ambiguous problems aligned with program goals

Think critically and move work forward without clearly defined next steps

Act as a researcher and advisor for special projects and cross-functional initiatives

Contribute to broader, interdepartmental projects/meetings

Support field teams, inbound questions, and project turnover coordination

Communicate statuses on operations and keep stakeholders informed of progress and roadblocks

Develop implementation plans and communications for BE/BEIs (Telehealth)

Provide details and insights during ACO REACH team meetings based on CMS communications, including reporting timelines and upcoming changes

Coordinate across departments to collect and submit SDOH data to CMS using the CMS-approved PRAPARE tool

Serve as a resource to the PTS team to assist with practice needs related to ACO REACH operations

Minimum Qualifications

Bachelor’s degree in a relevant field

3+ years of progressive healthcare experience

Thrives in a fast-paced environment and can manage multiple priorities

Familiar with project management tools such as Monday.com

Experience in data analytics using Excel or Google Sheets

Willingness to travel as needed to Aledade’s headquarters and other offsite meetings

Preferred Qualifications

Knowledge of CMS VBC programs

Ability to adapt communication style based on audience needs

Frequently and effectively collaborates cross-functionally

Exceptional written and verbal communications skills

Snowflake experience

Tableau experience

Self-starter with strong interpersonal skills

Proven ability to coordinate and facilitate new ideas into action while collaborating across multiple dynamic projects and teams

Physical Requirements

Sitting for prolonged periods of time. Extensive use of computers and keyboard. Occasional walking and lifting may be required
