Responsibilities:
Coordinate and direct care for a caseload of home health patients requiring physical therapy as the primary service.
Perform comprehensive assessment, planning, implementation, and evaluation of care plans for each patient.
Optimize scheduling and visit utilization to maintain productivity and best practice standards.
Conduct functional assessments using objective measures of activities of daily living and document results.
Develop individualized, realistic, patient‑centered goals in collaboration with patients, families, physicians, and other health‑care providers.
Reassess patients quarterly, compare new measurements to prior data, and document the effectiveness of therapy.
Observe and record clinical findings, report patient responses to the physician, and adjust the plan of care as needed.
Identify performance improvement initiatives and assist in designing or implementing home health standard‑of‑care programs.
Minimum Qualifications:
Bachelor’s Degree
Licensed Physical Therapist (PT)
Active American Heart Association BLS
Valid state driver’s license with current car insurance
Preferred Qualifications:
Master’s Degree
Doctorate Degree
Certification for Oasis Specialist‑Clinical (COSC)
