Overview Provide nursing care for a vulnerable adult population. Performs nursing assessment, treatment, education and health care service coordination to individuals in need. The Acute Care LPN collaborates with all components of the team and other health care agencies to ensure coordinated, comprehensive services.

All responsibilities are essential job functions.

Responsibilities

Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, nursing assessments, administering orders, medication reconciliation, taking and monitoring vitals, and documenting in the electronic medical record.

Render and supervise care of clients on the unit.

Monitor medications and consult with prescribers.

Provide appropriate health care and coordinate referrals as prescribed by providers.

Participate in the evaluation of a client’s condition and progress with team members.

Promote education and adherence to prescribed treatment protocols.

Facilitate wellness and medically related psychoeducational groups to individuals.

Record all client information within the electronic medical record system daily.

Assist RNs on a daily basis.

Offer nursing consults on emergency service evaluations.

Work with clients to coordinate care with their PCP, psychiatrist, and other professionals including medication refills and follow-up medical appointments.

Specialty Responsibilities:

Charge Nurse: as assigned by program leadership. When assigned, you are expected to maintain overall shift operation in a safe, efficient manner in the absence of management and supervise staff to ensure delivery of safe and effective patient care.

Report urgent issues on the unit to the management team and utilize on-call resources for emergencies, medical and disciplinary situations as needed.

Maintain regular, reliable, and predictable attendance.

Position Qualifications License/Certification/Education

Required:

Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.

Satisfactory CORI background check.

Driving is not a requirement.

Experience/Skills

Required:

Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.

Demonstrates ability to communicate effectively and professionally with clients, staff, and providers.

Preferred:

One (1) year of nursing experience.

Knowledge of medical, substance abuse, and mental health nursing.

Additional Standards

Unless certification, licensure or registration is required, an equivalent combination of education and experience may be substituted.

Department-specific competencies and their measurements will be developed and maintained in the departmental job description and reviewed with position incumbents.

Physically, work requirements include ability to move about, stoop, crouch, kneel or crawl; ability to hear and distinguish speech and alarms. Reasonable accommodations may be made to perform essential functions.

Physical Demands Work is considered medium; may be required to lift up to 25 pounds. Work is indoors in a patient care setting.
