Overview

The Hospice Nurse Practitioner - Exempt is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he upholds the Code of Ethical Conduct and promotes positive working relationships within the company, across departments, and with external stakeholders. The Nurse Practitioner provides care for patients in palliative care or hospice settings, including assessments, disease and treatment education, and symptom management to meet patient and family needs. The NP collaborates with the PCP, specialists, and the assigned Compassus Medical Director to ensure compliance with practice standards and codes. When providing hospice care, the NP functions as an extension of the medical director for the interdisciplinary team (IDT) and hospice physician to support routine recertification and emergency assessments, educational, and evaluative services. The NP may be required to provide on-call support and after-hours emergency visits/admissions. Position Specific Responsibilities

Perform routine and emergency assessments and recertification visits for patients entering their 3rd benefit period or beyond (including admissions and transfers). Record observations, treatments, and other pertinent information; communicate findings and management plans to the Medical Director, Attending Physician, DCS, and IDT. Provide physical and emotional care to patients and families in crisis; act as attending practitioner at admission when selected by the patient, coordinating with the IDT and physicians on symptom management and end-of-life care. Rotate into on-call schedule as required, with physician backup for admission certification. Palliative Care / Home Care Responsibilities

Every patient encounter may include routine and emergency assessments, bedside evaluations, diagnosis and management of acute and chronic conditions, and prescribing medications within scope of practice. Order and interpret laboratory and radiology tests; order treatments and durable medical equipment as indicated; provide urgent care as needed. Coordinate care with palliative care physician/designees and inform primary physicians of services provided; assist in all facets of care coordination for referrals. Prepare and maintain accurate patient records and documentation to support medical practice and reimbursement; ensure compliance with laws and Collaborative Agreements. Assist with goals of care, advanced care planning, and education for patients and families. Provide training and continuing education for staff; participate in development of clinical practice guidelines/standards. Adhere to patient confidentiality and organizational confidentiality policies. Collaboration & Communication

Collaborate with Medical Director, attending physicians, DCS, and IDT; communicate patient and family needs for clinical and psychosocial interventions; identify needs for intervention by IDT members; complete required paperwork within 24 hours of visits. Educate patients and caregivers about care, disease processes, dying process, symptom control, wound care, and other relevant topics. Participate in IDT meetings and communicate needs across teams; ensure timely clinical documentation. Education, Experience, and Certifications

Master’s degree in Nursing required. For hospice care: minimum of three (3) years nursing practice; hospice, oncology, home health, or long-term care preferred. For palliative care: minimum of three (3) years nursing experience; clinical nursing care in oncology, hospice, geriatrics, or intensive care preferred. Active CNS or Nurse Practitioner license in state of employment; NP certification in Adult, Family, Acute, or Geriatric required; Active NPI number; DE A certification if state allows. Board Certification in Hospice and Palliative Care preferred but not required. Skills

Mathematical, analytical, and problem-solving skills; strong written and verbal communication; organizational and time-management abilities; ability to travel; flexible schedule including nights and weekends; maintain confidentiality. Other Requirements

All states except Iowa require a current collaborative physician agreement. Physical Demands and Work Environment

The role requires standing, walking, manual dexterity, and ability to lift/move up to 50 pounds. Visual acuity, ability to work with bodily fluids, infectious diseases, and conditions typical to clinical care are expected. Regular use of standard clinical equipment is required.
