Overview

UW Medical Center - Northwest has an opportunity for a Nurse Care Coordinator to join the Care Management team. The goal is to achieve coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge. This position provides care coordination services for Medical Telemetry patients and works with the interdisciplinary health care team to align the patient’s plan of care and discharge planning. Sign-On Bonus:

Full Time Day Shifts Monday-Friday Primary Responsibilities

Identification and monitoring of high medical needs patients to ensure timely completion of consults, procedures and tests, ongoing team communication, and coordination between providers across the continuum of care. Lead medical team and nursing huddles and work to remove barriers to discharge due to medical complications and/or system or team obstacles. Assist with ordering complex medical equipment and ensure safe transitions of care. Develop and monitor outcome measures related to care coordination and staff and patient satisfaction with discharge planning; provide ongoing consultation and education to staff on achieving improved patient flow through the UWMC system. Qualifications

Bachelor's degree in Nursing or related field; Master's degree preferred. Three years of professional nursing experience; Care Management experience strongly preferred. Excellent verbal and written English communication, effective interpersonal and leadership skills, and clinical expertise. Ability to access information and document in electronic medical record. Current Washington State RN license at time of hire and current BLS certification. About UW Medical Center – Northwest
