Telephonic Nurse Case Manager II Elevance Health – Virtual full‑time, central Colorado. Eligible candidates must live within a reasonable commuting distance to an office for required in‑person training sessions. Remote work is primary; onsite location required at least once per week.

Location & Hours Month‑to‑month: Monday‑Friday 9:00 am‑5:30 pm EST; late‑evening shift 11:30 am‑8:00 pm EST. Hybrid virtual with periodic in‑person onboarding.

Key Responsibilities

Care management of members with complex, chronic needs, delivering outcomes across the continuum.

Conduct telephonic assessments to identify needs and develop individualized care plans.

Coordinate internal and external resources, including authorizations/referrals and benefit navigation.

Monitor plan efficacy, modify interventions, and maintain documentation.

Interface with Medical Directors, Physician Advisors, and behavioral health providers.

Assist with problem‑solving provider claims or service issues.

Contribute to utilization and care‑management policy development.

Minimum Requirements

BA/BS in a health‑related field and ≥5 years clinical experience, or equivalent combination.

Current, unrestricted RN license in applicable state(s); multi‑state licensure if providing services in multiple states.

Case management experience preferred; certification as a Case Manager highly preferred.

Preferred Capabilities

Telephonic case management experience in a Managed Care setting.

Advanced Microsoft Office or new computer program proficiency.

Strong critical thinking, communication (talk/typing), and email responsiveness.

Acute care setting or managed care experience.

Salary & Benefits Salary range: $80,608.00 – $120,912.00 (Colorado). Elevance Health offers a comprehensive benefits package, including 401(k) match, equity stock purchase, and incentive and recognition programs. Total rewards may vary by location, experience, education, and market conditions.
