Annual Compensation: $81,806 - $99,985 (Based on a full-time schedule).
On-Call and Weekend Pay are in addition to the base rate of pay mentioned above.

Responsibilities The
Registered Nurse (RN)
in Home Health provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Coordinates the total plan of care with other healthcare professionals involved in care, ensuring continuity by planning and exchanging information with physicians, agency personnel, patients, families, and community resources—all within a Point of Care setting. Makes initial and/or comprehensive nursing evaluation visits, determines the primary focus of care, develops the plan of care within state-specific guidelines, and submits accurate, complete, and timely documentation in collaboration with physician orders. Performs patient assessments at designated time points and develops appropriate plans of care. Ensures patients meet home health eligibility and medical necessity guidelines as defined by the payer source.
Education and Experience Current RN licensure in the state of Colorado. Current CPR certification. Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
