Fellowship Program Description Oak Street Health’s NP Fellowship in Adult Primary Care is a 12‑month paid fellowship in collaboration with the University of Michigan School of Nursing. The program follows a value‑based care model that rewards quality over volume. Fellows build their own patient panel, learn from multiple preceptors, and engage in didactic content that builds on, rather than repeats, graduate training. Fellows commit to working at Oak Street for at least one year after program completion; many choose to stay longer to improve the health of communities across the country. Cohorts start in March and September each year.

Requirements
Adult or adult‑gerontology primary care NP who graduated or will graduate from their training program between May 2024 and September 2025
Currently residing in or willing to relocate to the program location for at least the next two years
Have not yet worked as an NP
Minimum of one year of experience, within the last five years, working as an adult RN
Minimum GPA of 3.0 in NP program
Currently holds or will hold by 3/16/2026 an active, non‑probationary state NP license
A genuine passion for adult primary care and older adult patients
Team‑oriented, collaborative attitude
Commitment to an innovative, population‑health focused model
