The RN lead works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care. The RN systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The RN utilizes evidence to drive clinical decisions and practice related to individual and population health. The RN establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The RN considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The RN participates in continuous quality improvement initiatives and assumes responsibility for their professional development. As the leader of the nursing team, the RN lead makes decisions of how to best utilize the skills of the nursing team to provide excellent patient care. Essential Functions
Telephone and MyChart triage: according to the chief complaint, assess the severity of the patient\'s symptoms and guide the patient to the appropriate level of care (Scheduled visit, Same day slot when available, Urgent Care, or Emergency). Urgent Care Triage: according to the chief complaint, assess the severity of the patient\'s symptoms and guide to the appropriate level of care. If the patient is staying with urgent care, begin the note and transfer to provider for completion. If transferring to the Emergency Department, ensure appropriate communication and transport, and document encounter in full in the EMR. Address medication refill requests by directly refilling when allowable per protocol, pending prescription with correct pharmacy information and sending to the Provider team, or advising the patient that an appointment is needed for further refills. Review and triage lab/x-ray results and notify the physician or other healthcare provider of critical values. Make follow-up calls to high-risk patients to assess changes of status or to ensure that the patient sought the appropriate treatment. Coordinate transitions of care and schedule patients accordingly to ensure continuity of care. Call patients with abnormal results as directed by providers and report abnormal results to governing bodies as directed (e.g., STI reporting and lead reporting). Direct normal result communication to Medical Assistants. Provide patient education and document in medical records. Leadership: Assign daily and special project roles for all RN and LPNs. Monitor task completion and productivity of the team. Coach, mentor, and train all members of the team for success. Serve as a reference for all clinical and administrative questions within the team. 40% Leadership, 60% Clinical. Act as a resource to medical support staff and collaborate with medical staff in carrying out delegated patient care activities. Education And Experience
Graduate of an accredited nursing program. Currently licensed in the State of Wisconsin. Five years of general nursing in a clinic or hospital setting, and previous experience with patients of all age groups. Experience within a community health center and with Epic preferred. Leadership experience preferred. Additional Requirements
It is mandatory that all employees, employed providers and volunteers are vaccinated. All hired candidates must submit proof of vaccination prior to orientation.
