Assist patients in navigating post-acute care, aiming to positively impact patient outcomes and referral satisfaction. Develop patient-centered transition plans using evidence-based guidelines, protocols, and metrics, promoting quality and efficiency. Represent the area in strategic relationships with health systems, hospitals, physicians, and executive opportunities. Qualifications
Education and experience, essential
Graduate of an approved nursing, therapy, or social work program Licensed in the current practicing state Minimum two years of field experience Knowledge of federal, state, and local laws governing Medicare certified home health and hospice Basic technology skills, including mobile device operation Preferred
Registered nurse or physical therapist Three or more years of field experience Previous experience in home health or healthcare sales Requirements
Valid state driver’s license Automobile liability insurance Dependable transportation in good condition Ability to drive safely in all weather conditions Additional Information
