Overview Hospice Admissions RN, M-F Full Time, AMAZING TEAM. Join Canyon Home Care and Hospice as the Hospice Admissions RN. We run our branch from the bedside back with a team‑oriented culture. Two admits a day; M-F is full time. We use Matrixcare and have clinical leaders with field experience who are willing to jump in.

Responsibilities

Provides services in accordance with the plan of care.

Makes the initial evaluation visit, determines the immediate care and support needs of the patient, assesses eligibility and homebound status and develops the plan of care in coordination with other IDT members.

Regularly reevaluates the patient\'s nursing needs and revises the care plan to meet the patient\'s needs.

Initiates the plan of care and necessary revisions.

Furnishes services requiring substantial specialized nursing skill.

Initiates appropriate preventive and rehabilitative nursing procedures.

Prepares clinical and progress notes for each patient visit and summaries of care conferences in a timely manner per agency policy.

Coordinates services with other IDT members, physicians, other care providers, patient, and caregivers as appropriate.

Informs physician and other personnel of changes in the patient’s condition and needs.

Counsels the patient and family/significant others in meeting nursing-related needs.

Participates in in-service programs, supervises and teaches other nursing personnel.

Understands and adheres to agency policies and procedures.

Processes orders and notifies physician of patient needs and changes in condition; completes certification/recertification orders and discharge summaries.

Determines the amount and type of nursing needed by each patient.

Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker as needed.

Supervises LPN/LVN and HHA/CNA as per regulatory requirements; assigns home health aides to patients with written instructions.

Conducts patient care conferences and participates in peer review and quality management as assigned.

Participates in utilization review of medical records as assigned.

Gives total patient care as needed.

Takes on-call duty nights, weekends and holidays as assigned.

Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in timeframes defined by the agency.

Appropriately utilizes ICD-10 diagnoses and codes.

Maintains patient confidentiality and safeguards all patient information.

Attends staff meetings and patient care conferences as scheduled.

Completes documentation and paperwork in a timely manner per agency policy.

Reports incidents, variances, or complaints to the Administrator/Clinical Manager promptly.

Demonstrates competent performance of technical skills according to established procedures.

Adheres to agency standards and interprets and performs assigned responsibilities accurately.

Maintains acceptable attendance status per agency policy.

Reports incomplete work to the Clinical Manager.

Maintains professional appearance and a clean, well-groomed image.

Demonstrates effective time management and completes daily documentation; manages overtime as needed.

Maintains a clean and neat work environment.

Demonstrates sound judgment, critical thinking and decision making.

Maintains current CPR certification.

Performs other duties as assigned.

Qualifications

Graduate of an approved school of professional nursing and currently licensed in the state(s) where practicing.

One (1) year nursing experience, preferred.

Acceptance of philosophy and goals of the agency.

Ability to exercise initiative and independent judgment.
