Overview
Strive Health is seeking an RN Care Manager to support patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). This hybrid role is primarily telecommunication-based but may require on-site visits to offices, clinics, or patient homes. Reports to the Lead, RN Care Manager. What You’ll Do
Serve as a critical resource for patients, families, local care teams, and healthcare professionals to coordinate services and optimize care delivery. Craft personalized care plans addressing clinical and non-clinical needs. Conduct regular assessments (onsite and virtual) to detect medical or social issues and changes in the home environment; promptly refer matters to appropriate channels. Initiate proactive outreach to patients and/or clinics to evaluate status, offer assistance, and address inquiries. Manage a diverse patient panel, evaluating needs while adhering to NCQA standards for complex case management. Communicate updates to the care team and family members; identify gaps in understanding and provide tailored education and resources. Serve as a liaison with dialysis facilities; convey care plan updates and maintain accurate program software records. Generate and evaluate reports using Strive data platforms to identify trends and focus areas. Provide culturally sensitive care and foster collaboration among clinical team members. Utilize clinical judgment, analytical skills, and knowledge of case management to deliver high-quality care. Provide in-person patient care as needed, which may include standing, sitting, walking, pushing, pulling, and lifting. Minimum Qualifications
Associate’s degree from an accredited school of nursing. Current Registered Nurse (RN) license in the state of practice. 3+ years of related education, experience, or certification. Current BLS or CPR Certification required. Reliable transportation with an active driver’s license for travel across an assigned region (offices, clinics, patient homes). Internet connectivity with minimum speeds: 3.8 Mbps upload / 3.0 Mbps download (latency acceptable). Preferred Qualifications
3+ years in nephrology, ER, ICU, Cardiac, or Ambulatory Complex case management nursing. Certified Case Manager (CCM) certification preferred. Proficiency with Microsoft 365 Suite (Word, Outlook, Excel) for a hybrid-remote environment. About You
Experience developing strong patient and family relationships to support Strive Health’s care model. Experience proactively contacting patients/clinics to assess clinical and non-clinical needs. Proficiency with documenting and navigating electronic medical records (EMR) and care plan management. Annual Salary Range:
