Certified Home Health Physical Therapist
to join our team. Therapists provide a holistic approach, supporting residents’ physical, spiritual, emotional, and clinical needs. They implement residents’ personal care plans, focusing on residents’ strengths, preferences, and routines, while providing comfort, companionship, and belonging. We offer a $10,000 sign-on bonus for a full-time, experienced Home Health Physical Therapist. Our communities offer levels of care with specialty programs such as urinary incontinence, vestibular rehab, home safety assessment, movement disorders, and low vision. Note: Specific state regulations and requirements may apply and take precedence over the requirements outlined here. Responsibilities
Provide home health physical therapy services to Erickson Senior Living residents within the Charlestown campus, following residents’ care plans and coordinated with the interdisciplinary team. Implement personalized care plans that align with residents’ goals, preferences, and routines. Collaborate with onsite physicians and the interdisciplinary team to address residents’ physical and functional needs. Support residents across the continuum of care with a holistic approach, including comfort, companionship, and meaningful engagement. Qualifications
Must possess a valid and current state Physical Therapy license. Degree and training must meet current Medicare and state requirements. Ability to complete documentation to meet reimbursement and regulatory requirements. Therapeutic experience in older adults across multiple diagnostic groups is preferred. Please note that Charlestown is a 110-acre continuing care retirement community located in Catonsville, Maryland, part of a national network managed by Erickson Senior Living. Compensation: Rates range from $47.00 per hour to $50.00 per hour, based on years of licensure. Onsite physicians group dedicated to serving Erickson Senior Living residents. Interdisciplinary clinical team collaboration to address residents’ needs. Onsite medical centers providing wellness visits and sick care for employees over 18.
