Staff This role involves weekend home health visits throughout Schuylkill County, serving adult and geriatric clients on a per-visit basis. One year of clinical experience as a licensed Physical Therapist is required. You will be an integral member of a multidisciplinary team providing rehabilitative care and skilled nursing to clients to help them remain at home. Responsibilities for a PT - Physical Therapist

Make home visits to clients in designated geographic territories. Perform diagnostic tests and measurements (mobility, joint range of motion, transfer status, stability, ambulation, strength and endurance, balance, and safety). Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents. Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions. Assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan as appropriate. Educate clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independent living. Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Qualifications for a PT - Physical Therapist

Current Pennsylvania PT license. Minimum of one year of recent work experience as a Physical Therapist. Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills. Solid computer skills; prior experience with electronic medical records (EMR) preferred. Benefits & Workplace Information
