Role Overview:
Valley Cities and community partners are dedicated to constructively addressing the opioid epidemic. The Nurse Care Manager is an essential member of a team of substance use disorder, behavioral health, primary care and other partnered providers who together will provide access to a continuum of services for persons with opioid use disorders, utilizing a “hub and spoke” model and a harm-reduction philosophy of care. This Certified Medical Assistant (CMA) position will provide assessment, education, stabilization, MA care, care planning and support to clients and medical support to prescribing providers and will be supported by a co-located full-time care manager.
Key Responsibilities:
Provide CMA support to Medication Assisted Treatment prescribing providers. Conduct screening of clients interested in medication assisted treatment. Provide client education about medication assisted treatment, Hep C and HIV treatments. Assist with enrollment and education of new Match clients. Provide screening and referral for Hep C, HIV, and smoking cessation. Obtain vital signs, urine drug screens and basic labs as indicated. Document in the electronic health record and treatment registry. Administer Vivitrol and manage follow-up care/scheduling. Participating in stabilization and maintenance phases of medication assisted treatment of clients. Provide ongoing coordination of follow-up care for clients utilizing Sublocade.
What We're Looking For:
Education:
Active Washington State Certified Medical Assistant credentials required. Current CPR certification required. Bachelor’s degree preferred. Washington State SUDP License preferred. Pass required background checks.
Experience:
Experience with case management or care coordination preferred. Experience with psychiatric and substance use disorder client populations preferred. Three years or more of clinical experience preferred.
