is seeking a passionate Primary Care Nurse Practitioner (NP) to join our team at the Veteran Affairs (VA) Community Based Outpatient Clinic (CBOC) in Cookeville, TN. Position Summary The Primary Care Nurse Practitioner diagnoses and treats diseases and injuries and delivers preventive and therapeutic care for Veteran primary care needs, including chronic disease management through routine history and physical examinations, testing, and non-invasive treatments. The NP collaborates with the Patient Aligned Care Team (PACT), including Primary Care Provider, RN, and Medical Assistant, and with the expanded PACT Team (family/caregiver, VA, and community-based services) to develop the patient care plan. The NP works under supervising physician direction in accordance with state licensing requirements. Schedule Monday through Friday, 8:00 AM – 4:30 PM, with no nights, on-call, or weekend requirements. Benefits Great work-life balance Generous time off package, including almost 7 weeks of cumulative time off across different banks Support from a PACT team (RN, LPN, MA) with 12–14 patients per day Competitive pay based on experience with regular market checks Medical/Dental through Blue Cross Blue Shield of Texas and Vision options Annual performance reviews to discuss goals and raises Annual incentive plans for primary care physicians and mid-level providers Half a day of dedicated admin time per week Strong clinic management, HR, Recruiting, and executive support Core Responsibilities Actively diagnose and treat veterans under the direction of a supervising physician Examine patients for symptoms of organic or congenital disorders Develop and implement patient management plans and provide continuity of care Order and perform diagnostic tests (e.g., x-ray, EKG, labs) and interpret results; provide follow-up as needed Counsel patients on medication use, educate patients, assess mental health, and provide routine health maintenance Evaluate external medical records and coordinate with VA rules for co-managed care Complete clinical reminders at the time of each patient visit Document medical records within 24 hours of encounters Comply with VA guidelines for timely responses to requests, alerts, consults, orders, labs, and diagnostics Follow VA formulary processes and consult protocols Ensure timely clinical documentation and complete encounters by end of business Meet VA and company training requirements Fulfill compliance requirements (OIG, JC, EOC oversight, lab, and related items) Aim for excellent clinical outcomes per VA guidelines Provide excellent customer service to veterans and clinic staff Participate in clinic outreach events to support enrollment initiatives Assist patient enrollment and retention by guiding scheduling and follow-ups as needed Embrace and support new clinical or operational initiatives Qualifications Graduate of an accredited nursing program with MSN required, including preceptorship Minimum two years of clinical experience in a related primary or ambulatory care setting in the last four years Credentialed and in good standing with the Veterans Health Administration (VA) Current, unrestricted license in the state of employment Current Basic Life Support (BLS) certification Current, unrestricted Drug Enforcement Administration (DEA) registration Strong computer and EMR skills Valor Healthcare is a veteran- and physician-led provider delivering high-quality care to veterans and federal government personnel.
