Nurse Practitioners provide primary and/or consultative professional care in a variety of care settings, functioning in collaboration with physicians according to a Collaboration Arrangement. Focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life. Conduct medical evaluations of predominantly elderly, frail, seriously ill patients in patients’ home environments. Determine follow up assessments according to patient needs and best practice guidelines. Communicate significant findings and changes to the clinical team, patients, families, facilities and other care personnel. Assess health status by obtaining relevant health and medical history and perform physical exams as per the Standard Care Arrangement. Order, interpret and evaluate laboratory tests and other diagnostic studies. Prescribe and monitor medications as allowed by prescriptive authority and state regulations. Develop treatment plans in collaboration with patients and caregivers. Refer patients to physicians or specialists as appropriate. Ensure continuity of care by liaising with the multidisciplinary care team. Provide patients and families with understanding of diagnosis and progression to reduce unnecessary ER and hospital visits. Hold case conferences with families when appropriate to advance the care plan. Meet minimum productivity requirements based on practice standards. Qualifications
Graduate of an accredited nursing program (Certificate, Diploma, Associate or Baccalaureate). Master's or doctoral degree as a Board-C Certified Nurse Practitioner. Current State License as a Registered Nurse and Nurse Practitioner. Nursing skills as defined by accepted standards. Ability to sit, stand, bend, lift and move intermittently; ability to lift at least 35lbs. Current and valid driver's license with reliable transportation and auto insurance. Demonstrates ability to assess and respond to needs of patients, families and caregivers in skilled-nursing facility settings. Ability to handle multiple priorities, documentation requirements and deadlines. Desired Qualifications
2 years of clinical experience in a post-acute setting or as a Nurse Practitioner (preferred). Experience with geriatric patients (preferred). Familiarity with Aprima EMR (preferred). Location
