We are seeking an Advanced Clinical Pharmacist Med Rec to ensure that we continue to provide all patients with high quality, efficient care. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of their work, serves as a patient advocate on an interdisciplinary rounding team, and collaborates with other healthcare professionals. Adherence to regulatory and medication safety guidelines is essential. The pharmacist will work with the certified pharmacy technician to review home medication lists and support a streamlined medication reconciliation transition into the hospital. Responsibilities
Provides leadership in managing patient-centered medication reconciliation on admission and discharge. Ensures medication history and reconciliation is completed according to established policies, procedures and protocols. Continuously works with the pharmacy services team and interdisciplinary care team to improve medication history and reconciliation processes. Provides training for and assures the continued competency of those involved in completing medication history and reconciliation. Meets with the certified pharmacy technicians regularly to provide leadership, share best practices, and provide coaching to enhance performance. Identifies and resolves drug-related problems. Verifies medications documented by the Certified Pharmacy Technician/Medication History Technician in the electronic health record and clarifies questions as needed. Prepares and dispenses medication orders per physician request according to policies and procedures. Communicates with providers to reconcile the admission medication history with formulary-appropriate alternatives and drafts the admission medication list for the provider. Documents interventions in the electronic health record and in CPW/VigiLanz; cosigns technician notes as needed. Reconciles discharge medications, provides recommendations to the provider, and drafts the discharge medication list. Performs discharge counseling for patients at risk of readmission and for patients and caregivers managing medications at home. Obtains language assistance as needed and provides verbal and written medication education to enhance understanding. Identifies and resolves drug-related problems that may affect patient adherence. Analyzes data to demonstrate the success of the pharmacy-led medication reconciliation safety program and monitors key metrics. Documents good catches and medication events; self-evaluates time spent outside direct patient care; assesses provider and nursing satisfaction for continuous improvement. Precepts pharmacy students and PGY1/2 residents; provides education and training to pharmacy staff and interdisciplinary team. Verifies CPOE orders for accuracy and appropriateness, intervening when unclear or unsafe. Interprets medication orders and maintains accurate patient medication profiles; troubleshoots as needed. Monitors medication therapy regimens for contraindications and interactions and supports pharmacokinetics, renal dosing, and other protocols. Participates in drug therapy management protocols and reviews culture and susceptibility data for antibiotic appropriateness. Maintains formulary and provides drug information to staff; uses Vigilanz/CPW alerts appropriately. Participates in quality improvement and medication-use review activities; documents and maintains logs and records. Leads targeted programs and medication use evaluations to improve outcomes while controlling costs. Supports facility regulatory and accreditation activities and adheres to Code of Conduct and Mission and Value statements. Qualifications
B.S. or PharmD in Pharmacy Required Current pharmacist licensure Required National Provider Identifier (NPI) Required Graduate of an accredited residency program Preferred 3-5 years hospital pharmacy experience with supervisory experience Preferred PGY1 Pharmacy Practice Residency Preferred PGY2 Specialized Residency Preferred HCA Florida North Florida Hospital is a 523-bed, full-service medical and surgical acute care center.
