Family Coach/Case Manager
to join our Family Hope Center. As a
Family Coach , you will support program participants by assessing their physical, social, emotional, and spiritual needs. You will provide family coaching (case management) services to Family Hope Center clients and assist individuals and families with issues such as poverty, drug addiction, child and spousal abuse, and homelessness. You will organize support groups, help clients obtain community services, refer clients to other community agencies, and collaborate with civic, religious, and business groups to address social problems through community programs. What you'll do: Create, develop, and implement Family Hope Center programs aligned with approved core programs and the Family Strengthening Framework to meet the physical, emotional, social, and spiritual needs of individuals/families as outlined in each person’s plan of service. Assist with design and coordination of supportive services for families in accordance with program goals and objectives. Assist eligible program participants with out-of-home placement or placement in respite care with family and/or friends when necessary. Timely and accurate intake and referral information analysis to assess clients/families’ needs; complete enrollment and required enrollment/assessment paperwork. Provide case management services including reviewing, evaluating, and updating individualized service plans, case notes, financial distributions, and referrals. Coordinate services for group and individual therapy for assigned families/clients as appropriate; provide counseling for crisis intervention as needed. Assist program participants with accessing community resources and program enrollments. Develop professional relationships with institutions and agencies that provide education, job training, living skills training, employment services, housing, and child care to coordinate services for participants. Analyze program needs; develop and implement changes to meet evolving client needs. Create and manage schedule of events and classes; coordinate and facilitate meetings and trainings for clients and groups, including Substance Abuse, Parent Education, Conflict Resolution, and Anger Management. What you’ll bring: Bachelor’s Degree in Social Work or a related field required. Minimum of 2 years prior related case management experience with at-risk children and families required. Proficient working knowledge of and/or experience using community resources. Ability to read, write, and speak Spanish professionally. Ability to drive assigned vehicle(s) or personal vehicle with appropriate state license; must provide proof of liability insurance and be eligible to be insured under Buckner’s policy. Must be age 21 or older to drive on behalf of Buckner. Ability to use up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people. Ability to walk, stand, and sit, sometimes for prolonged periods.
