Monogram Health seeks skilled Nurse Practitioners and Physician Assistants committed to making a difference in patients' lives. As a key member of an integrated Care Team, you will help patients with multiple serious diseases improve their quality of life and slow kidney disease progression. Your Impact
You will provide personalized, compassionate care primarily for patients with CKD and/or ESRD/ESKD, maintaining accurate records, scheduling follow-ups, and administering care. Conduct assessments, including wellness exams in-home and virtually Educate patients and families about health benefits and programs Document medical information thoroughly Coordinate care with healthcare providers Complete all documentation timely Maintain quality care standards This role is remote within the employed state, with occasional training outside the state Prescribe medications, order tests, collaborate with physicians Participate in care team meetings and educational initiatives Assess health status, develop and implement treatment plans, follow-up on patient progress Perform other duties as assigned Position Requirements
Bilingual (English/Spanish) preferred Active RN and Nurse Practitioner or Physician Assistant licenses Board certification (ANCC/AANP for NP, NCCPA for PA) Current DEA certification Ability to work independently Valid driver’s license and transportation Proficiency with medical instruments and data management systems Effective communication skills Knowledge of federal and state regulations (HIPAA, OSHA) Minimum 2 years of patient care experience Managed Care/IPA/Health Plan experience Experience with wellness visits, virtual or in-home Additional Information
