As a registered nurse care coordinator (RNCC) at UAB Medicine, you will join a Magnet-designated nursing team to provide high-quality care for complex cases. The UAB Care Transitions Department includes more than 200 registered nurse care coordinators and social workers who are integral members of the interdisciplinary team. RNCCs provide care in inpatient and ambulatory settings, including the Emergency Department, with a focus on screening and assessing patients for post-discharge needs, providing referrals for in-home care, and participating in care coordination and discharge planning across the continuum of care. The clinical ladder program offers annual professional development opportunities that may increase compensation. Nurses have access to free monthly continuing education opportunities to help fulfill licensure requirements. Work Environment

To coordinate, negotiate and obtain services and resources for the care of patients. To review case criteria to determine medical necessity for admission and continued stay. To provide discharge planning and care coordination that identifies the most cost effective methods of patient care. Qualifications

Bachelor's degree in Nursing with three (3) years of related experience required, or an Associate’s degree in Nursing with five (5) years of related experience required. Work experience may not substitute for education requirement. Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required. Certified in Basic Life Support (BLS) from the American Heart Association (AHA) required or must be obtained within 30 days of employment. Depending on the unit/department, Advanced Cardiac Life Support (ACLS) certification and/or Pediatric Advanced Life Support (PALS) may be required. Details
