has a full time opportunity for an experienced
Physical Therapist, PT
performing
home health visits
with our Davidson County Visits office in North Carolina. This office services adult and geriatric clients on a per visit basis, in the comfort of their own homes.
Coverage area includes Thomasville, Trinity, Archdale, Sophia . Responsibilities for a PT - Physical Therapist
Make home visits to clients in designated geographic territories. Perform diagnostic tests and measurements, such as mobility and range of motion, transfer status, stability, ambulation, strength and endurance, balance testing, and safety assessments. Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions. Continually assess and revise the Physical Therapy care plan, and participate with nursing in the multidisciplinary care plan, as appropriate. Educate clients, family members, or other representatives in rehabilitative care and activities necessary to promote health, safety, and independence. Accurately document observations, interventions, and evaluations pertaining to client care management using a state-of-the-art touch pad tablet. Qualifications for a PT - Physical Therapist
A current North Carolina Physical Therapist (PT) license. Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills. Solid computer skills; prior experience with electronic medical records (EMR) preferred. Benefits
