$20,000 (Full-time PTs) BAYADA Home Health Care is seeking a Physical Therapist (PT) for a full-time position with our new Central NJ Senior Living team – a Medicare Certified home health office. This office provides care to adult and geriatric clients in communities such as assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities in Somerset County. This is a hybrid position
that may include seeing clients in the traditional home health community setting. Responsibilities
Make home visits to clients in designated geographic territories. Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments. Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents. Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions. Continually assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan as appropriate. Educate clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independent living. Accurately document observations, interventions, and evaluations pertaining to client care using a state-of-the-art touch pad tablet. Qualifications
Current NJ Physical Therapist (PT) license. Minimum of one year of recent PT experience. Graduation from an American Physical Therapy Association-approved program or AMA Committee on Allied Health Education and Accreditation-approved program, as indicated by transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills. Solid computer skills; prior EMR experience preferred.
