Nurse Case Manager II Contract Duration:
3+ months Summary
The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy to meet an individual’s benefit plan and health needs through communication and available resources to promote optimal cost‑effective outcomes. Duties
Conduct comprehensive assessments of referred member's needs/eligibility and determine approach to case resolution. Apply applicable criteria and guidelines, standardized case management plans, policies, procedures, and regulatory standards while assessing benefits and member’s needs. Administer benefits and utilize case management and quality management processes in compliance with regulatory and accreditation guidelines and company policies. Experience
3 years clinical practice experience (e.g., hospital setting, home health, ambulatory care). Healthcare and/or managed care industry experience. Preferred case management experience; proficiency with computer skills navigating multiple systems and keyboarding. Effective verbal and written communication skills. Ability to multitask, prioritize, and adapt to a fast‑paced changing environment. Education
RN with current unrestricted state licensure. Seniority Level
