Job Summary and Qualifications

We are seeking a Registered Nurse (RN) Case Manager who will provide clinical expertise to ensure all patients receive high-quality, efficient care. You will use your knowledge and skills to positively impact patient care. What you will do in this role

Coordinate the plan of care for the patient stay, manage length of stay, ensure appropriate resource management, and develop a safe discharge plan. Facilitate progression and transition of care; identify patients at risk for adverse outcomes during transitions between levels of care. Perform a comprehensive assessment of psychosocial, medical and discharge needs for patients and families; reassess clinical condition as indicated. Ensure appropriate post-acute medical needs and level of care in partnership with Social Services. Evaluate progression of care using evidence-based tools and approved criteria (InterQual); escalate progression and transition issues as needed. Make referrals to third-party payer, disease and case management programs for recurring patients and those with chronic diseases. Act as a liaison through effective communication with physicians, patients/families, hospital staff, and outside agencies; advocate for patient rights and preferences. Qualifications

Graduate of an accredited nursing program; Associate’s Degree in Nursing required, BSN preferred Licensed RN in the practicing state or active Compact/Multi-State license 1-3 years of clinical, hospital nursing experience required 2 years in case management or critical care preferred Certification in case management or utilization review preferred InterQual experience preferred Memorial Health University Medical Center has provided quality healthcare since 1955.
