(PRN) Speech Therapist (Montgomery, AL) - ProHealth Home Health & Hospice
Responsibilities
Implements and administers Speech Language Pathology services as ordered by physicians.
Directs and supervises personnel as assigned; participates in care planning with home care team.
Takes initial history and performs initial evaluations; conducts all skilled procedures.
Consults with physicians regarding changes in treatment; writes progress reports to physicians.
Instructs patients and family members in home programs; presents in-service sessions as needed.
Completes initial evaluation, treatment plan and goals; obtains physician approval; maintains ongoing recertifications (every 60 days if continued).
Writes progress notes after each visit; performs reevaluations and discharge summaries as applicable.
Develops and revises the plan of care; assists in determining level of function and revises as necessary.
Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant changes within agency timeframes.
Utilizes ICD-10 codes appropriately; participates in QAPI activities as assigned.
Qualifications
Master’s or doctoral degree in Speech Language Pathology, and licensed as a Speech Language Pathologist by the state of practice, or
Completed 350 clock hours of supervised clinical practicum (or in process), at least nine months of supervised full-time SLP experience, and successfully completed a national examination.
Currently licensed in the state(s) where practicing.
Two (2) years’ experience preferred.
Working Environment Works indoors in the agency office and patient homes; travels to/from patient homes.
Lifting Requirements
Ability to participate in physical activity; ability to work for extended periods standing and engaging in physical activity.
Moderate lifting; ability to do extensive bending, lifting and standing on a regular basis.
