Overview
RN - Oncology Nurse Navigator is responsible for independently planning and providing professional navigation assessment and patient/family education, and coordinating care for cancer patients in active treatment. Provides nursing services within the scope defined by the North Carolina Board of Nursing and collaborates with a multidisciplinary team to deliver age-appropriate care. Responsibilities
Coordinate communication among multidisciplinary team, patient navigators, and healthcare providers to deliver patient/family centered care. Apply clinical judgment to expedite appropriate care and treatment, planning and evaluating patient care with the interdisciplinary team. Facilitate continuity of care using the nursing process and coordinate health services across the care continuum, including telecommunication with patients and families. Educate and advocate for cancer patients and families, assisting them to navigate the cancer care system and engage appropriate community resources and outside agencies as needed. Position Details

BSN from an accredited School of Nursing (required) Two years of oncology nursing experience (required) Patient navigation experience/certification preferred Current RN license in North Carolina Basic Cardiac Life Support (BCLS) required Essential Functions
Coordinate communication between the care team and patient/family; provide patient/family centered care and support for cancer care navigation Use clinical judgment to plan and expedite patient care; participate in interdisciplinary planning Ensure continuity of care across the care continuum; establish long-term patient relationships as needed Promote patient safety and quality through education, advocacy, and coordination of services Document care encounters and collaborate on survivorship planning and transition Adhere to the ANA Code of Ethics and pursue ongoing professional development Participate in governance, standards of care, and peer review to promote excellence About Advocate Health
