Job: Occupational Therapist (OT) Home Health - Full Time Location: Contra Costa County, CA Overview
ABL Health Care is seeking a compassionate, organized, and team-oriented Occupational Therapist (OT) to join our Home Health team in Contra Costa County, CA. This is a full-time position working with a dedicated team of healthcare professionals to evaluate patients, create and implement care plans, and monitor progress in the home setting. Responsibilities
Provide, coordinate and oversee skilled intermittent care to patients in the home, including instruction and education to promote maximal functional status. Regularly evaluate and re-evaluate patient status. Administer treatments, monitor patient conditions, and report signs/symptoms to Physician, Case Manager, and Director of Nursing as needed. Initiate, develop, implement and revise the patient\'s plan of care (POC). Address all problems identified in the POC. Provide educational information to patient/family/caregiver regarding the condition. Communicate with other disciplines involved in the case as warranted. Maintain accurate clinical records and submit documentation using a company-provided device. Comply with all company policies and procedures to ensure high standards of care. Qualifications
Current OT License to practice in California – Required Experience in Home Health or Hospice care – 1 year required Current CA Driver\'s License Able to travel within a 15-25 mile radius on a regular basis Benefits (Full-Time)
