The Physical Therapist (PT) at Elmwood Healthcare - Home Health will be responsible for evaluating, planning, and implementing personalized therapeutic interventions to improve clients' physical function, mobility, and overall well-being. Working directly in clients' homes, you will provide hands-on therapy, education, and support to help clients achieve their rehabilitation goals and enhance their quality of life.

Key Responsibilities:
Assessment and Evaluation:
Conduct comprehensive assessments of clients’ physical abilities, mobility, strength, balance, and coordination to identify areas for improvement and develop individualized treatment plans. Treatment Planning:
Design and implement personalized therapy plans that address clients’ specific needs, goals, and functional limitations. Adjust plans as needed based on client progress and feedback. Therapeutic Interventions:
Provide direct therapy services, including exercises, manual therapy, gait training, balance and coordination activities, and modalities to enhance physical function and mobility. Client and Family Education:
Educate clients and their families on exercise techniques, home safety modifications, pain management strategies, and ways to support and maintain physical progress outside of therapy sessions. Documentation:
Maintain accurate, timely, and thorough documentation of therapy services, client progress, and outcomes in compliance with company policies, insurance requirements, and regulatory standards. Collaboration:
Work collaboratively with other healthcare professionals, such as physicians, occupational therapists, and nurses, to coordinate comprehensive care and ensure continuity of treatment. Case Management:
Monitor client progress, modify treatment plans as necessary, and provide regular updates to the interdisciplinary team and clients’ families. Compliance and Safety:
Ensure that all therapy practices adhere to regulatory standards, safety protocols, and best practices within the home healthcare environment. Direct Patient Care / Clinical Skills:
Medication Management, Education and Reconciliation Development and Teaching of Individuated Home Exercise Programming (HEP) Strategic Therapies to Address Movement, Function and Strengthening Gait, Mobility and Ambulation Training Physical Reconditioning Range-of-Motion (ROM) Training ADL/IADL Training Qualifications
Education:
Master’s or Doctoral degree in Physical Therapy from an accredited program. Licensure:
Current and valid Physical Therapist license (PT/DPT) in the Commonwealth of Massachusetts. Experience:
Minimum of one (1) year of experience as an Occupational Therapist, preferably in a home healthcare setting. Skills:
Strong clinical assessment and therapeutic skills, excellent communication and interpersonal skills, ability to work independently and manage time effectively. Certifications:
CPR Certification with Basic Life Support (BLS) or Advanced Life Support (ALS) is required. Other Requirements:
Valid driver’s license, reliable transportation, and willingness to travel to patients' homes within the designated service area.
