Responsibilities
The
Physical Therapist (PT, DPT, RPT)
in Home Health is responsible for assessing patient needs related to mobility, endurance, pain, and other medical conditions. Based on assessments, the therapist develops treatment plans, performs interventions to improve patient well-being, and evaluates progress. Evaluate the patient's functional status and therapy needs. Assist in developing the plan of care with the physician. Monitor and report patient responses and condition changes to the healthcare team. Treat patients to relieve pain, restore or develop function, and maintain maximum functioning according to standards of practice. Educate patients, families, and healthcare team members about the care plan, limitations, and progress.
Education and Experience
Licensure Requirements Current Physical Therapy license in the state of practice Current CPR certification Valid driver's license, vehicle insurance, and reliable transportation
