This is a full time position.
Duties
Participate as a member of the multidisciplinary Assertive Community Treatment (ACT) Team providing comprehensive community-based rehabilitative mental health treatment including case management and coordination of services to adults with a severe and persistent mental illness from underserved communities. The person is this position will serve in a dual role as a case manager and vocational specialist. As a case manager, they will consult and work with team and other community agencies and families to maintain needed resources and coordinate services in the treatment process. Assure client has contact with their psychiatrist and following up to monitor that client is taking prescribed medications. The vocational specialist is a key team member in the service planning for clients who want to work or are currently working. They provide supported employment & education services. Core services include the following: (1) Engagement; (2) Vocational assessment ;( 3) Job development; (4) Job placement (including going back to school, classes) ;( 5) Job coaching & follow-along supports (including supports in academic settings); and (6) Benefits counseling. Additional responsibilities: Completing functional assessments. Engage with individual clients and families. Develop professional relationships and partnerships with other community service providers. Provide community engagement or program outreach as required. Offer training and education to help clients learn the necessities of daily living including self-care, money management, social skills development, use of public transportation, and securing employment.
Job Qualifications
Bachelor’s degree in Behavioral Sciences or Related field required. Experience may not be exchanged for education. Minimum 1 year experience in family outreach, support, counseling or community mental health. Minimum 1 year of experience providing employment services (e.g. job development, job coaching, supported employment). Must meet mental health practitioner requirements of law. Must be fluent in English and Hmong. Valid Driver's License, auto insurance and reliable vehicle required.
