RN - Case Manager - Midwest City at SSM Health. Location: St. Anthony’s Hospital – Midwest City, 2825 Parkland Dr., Midwest City, OK 73110. Department: System Case Management. Schedule: 8am-2pm (any four days between Monday and Friday). Employee type: Regular. Position Summary: St. Anthony’s Hospital in Midwest City is seeking a dedicated and compassionate Case Manager to join our System Case Management team. Coordinate patient care and discharge planning Collaborate with interdisciplinary teams to ensure quality outcomes Advocate for patient needs and connect them with appropriate resources Maintain accurate documentation and compliance with hospital policies Qualifications

Assesses, develops, plans, implements, and evaluates patients discharge planning needs to next level of care in collaboration with interdisciplinary team. Ensures compliance with Condition of Participation (CoP) discharge planning. Identifies and escalates gaps/barriers related to inpatient and observation length of stay. Assesses patients at high risk for readmission; develops plans and collaborates with multidisciplinary team regarding discharge planning to next level of care. Participates with other team members regarding opportunities for improvement in standard work. May also be responsible for utilization management, ED case management, admission process, and liaising with post-acute providers (SNFs, rehab, home health, etc.). Applies evidence-based practice and nursing ethics to patient care; adheres to age-specific and clinical needs per department scope of service. Demonstrates professional nursing standards and uses the ANA Code of Ethics as guidance. Performs other duties as assigned. Education
Graduate of an accredited school of nursing or nursing education equivalency for licensing Experience
Three years of acute care experience Physical Requirements
Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs Frequent sitting, standing, walking, reaching; repetitive movements Visual and depth perception requirements; color differentiation Hearing and speech for communication; alarms and equipment awareness Frequent keyboard use; occasional bending, stooping, kneeling, squatting, twisting Occasional lifting 25-50 lbs; rare climbing Licenses and Certifications
Registered Nurse (RN) – Illinois, Missouri, Oklahoma, Wisconsin licensing as applicable by state of work location via compact or state board Work Shift: Day Shift (United States of America). Job Type: Employee. Department: System Case Management - OK. Scheduled Weekly Hours: 24. Benefits
