Overview
Full Time Occupational Therapist (OT) – Home Health. Territory: San Mateo County: Redwood City and surrounding cities. 21st Century Home Health Services (21HHS) is the largest home health agency in San Francisco and a growing Bay Area provider. We care for patients across multiple counties and are actively expanding into Marin and Sonoma counties. All opportunities require being in the field visiting patients in their homes. Remote/work-from-home positions are not available unless specifically noted. Base pay range: $120,000.00/yr - $130,000.00/yr. Actual pay is based on skills and experience; discuss with your recruiter to learn more. Wage range: Occupational Therapist: $120,000.00 - $130,000.00. In partnership with the Case Manager, provide skilled OT services in patients’ homes to evaluate and address needs so patients can live safely and productively at home. Areas may include home safety assessment, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease/dementia management, and caregiver training. Serve as an educator, patient advocate, and member of the interdisciplinary care team. Qualifications
Current unrestricted CA Occupational Therapist License. Current BLS/CPR Certification. Valid CA Driver's License. Knowledge of local community resources; understanding of development, behavior, and culture in health care. Knowledge of homecare regulatory guidelines (state and federal) is preferred. Home health care experience is highly preferred; high-acuity experience is a plus. Experience working with an interdisciplinary team; case management fundamentals are highly desired. Self-directed, empathetic, critical thinker with outstanding interpersonal and organizational skills; leadership skills preferred. Prior to scheduling an in-person interview, candidates must complete a computer literacy and competency assessment to ensure comfort with electronic charting and company systems. As a member of the care team, you will evaluate and address needs of individuals in their homes to support safe, productive living. This includes patient education and caregiver support, in collaboration with the Case Manager and other clinicians. Benefits and Perks
