The primary role of the Senior Home Infusion RN is to provide clinical care and education to home-based patients receiving a wide range of parenteral medications dispensed from Optum Infusion Pharmacy. Medications may be administered intravenously, subcutaneously, intramuscularly, or intrathecally. Service delivery occurs in patients’ homes and in ambulatory infusion suites, where available. All infusion services and medication delivery follow OIP service policies, procedures and therapy-specific protocols based on orders from a physician or licensed practitioner. The Senior RN collaborates with the multi-disciplinary team to prepare, deliver and seek reimbursement for infusion therapy that is safe and effective for each patient. Additional duties may include staff training and orientation, medical record and process auditing, plan of treatment oversight, and completion of supervisory nursing visits. Primary Responsibilities

Establish and maintain positive clinical rapport with patients and families, providing infusion therapy, self-care and self-administration education, and clinical monitoring of therapy response Assess patient suitability for home infusion therapy, including home safety, caregiver support, and patient/caregiver independence in therapy administration and monitoring Obtain and document patient medical history at the start of care and ongoing related to the diagnosis treated with infusion therapy Complete required clinical documentation thoroughly and timely (nursing plans of care, plans of treatment, visit notes, clinical summaries, medication profiles, discharge summaries, and updates to the care team), and obtain signatures on relevant forms Participate in case conferences to plan patient care with the multi-disciplinary team Provide comprehensive skilled care to patients of varying ages and medical conditions as outlined in the plan of care and physician orders Establish, secure and maintain vascular access devices as prescribed (PIV, PICC, tunneled CVC dressing changes, port access) Administer medications per company policies and protocols, including vital signs, infusion reaction management, side effects monitoring, and specimen procurement as needed Communicate observations of patient response to therapy with the care team in a timely manner Establish collaborative relationships with home health agencies, providing education and support as requested Maintain knowledge of infusion therapies and adhere to company policies and training resources Maintain licensure in all states served and comply with state and federal regulations (OSHA, HIPAA, and home infusion standards) Complete required training, including CPR certification, upon hire and annually Represent the company professionally when educating external customers, payers and referral sources Participate in staff orientation and training; act as a clinical educator and resource Conduct supervisory nursing visits, document nurse competency, and provide ongoing education Assist with timely signing and processing of physician orders (Plan of Treatment and Supplemental Orders) Participate in medical record and performance-improvement audits as assigned May work any day or shift as needed to accommodate patient schedules, including after-hours on-call May transport, lift or carry equipment to patient homes and travel as required May drive a personal or rental vehicle as needed for patient and care-site visits Other duties as assigned Required Qualifications

Active and unrestricted RN license in the practicing state(s) and any others as applicable 2+ years of recent direct patient care experience as an RN in a home infusion or ambulatory infusion setting Strong teaching, organizational and critical thinking skills Effective interpersonal communications, both written and verbal Proficient with Microsoft Office and electronic medical records (EMR) Knowledge of Infusion Nursing Society (INS) standards and procedures Ability to work with an interdisciplinary team and independently in home or alternate-site settings Flexible schedule and reliability including attendance, overtime and after-hours on-call Access to reliable transportation for travel to patient sites Preferred Qualifications

Knowledge of HIPAA, OSHA, and ACHC accreditation standards for home health and home infusion Remote work eligibility with adherence to UnitedHealth Group Telecommuter Policy Pay is based on multiple factors including location, experience, and qualifications. In addition to salary, benefits include a comprehensive package, incentive programs, equity stock purchase and 401k contribution. The hourly pay range for this role is $34.23 to $61.15 per hour (full-time). We comply with all applicable minimum wage laws.
