You will ensure high quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes As an active member of the rehab team, you will provide, coordinate, and direct care specific to the needs of each Rehab patient You will collaborate with the Rehab Program Director and Facility Case Management Director regarding departmental functions, and coordinates efforts within the rehabilitation team You will provide thorough verbal and/or printed information to help patients/families/caregivers make informed decisions about post-acute care options, while addressing goals of care and treatment preferences You will facilitate discharge and arrangement of follow-up services, implementation of discharge/transition recommendations, and identify and utilize appropriate community resources to meet patient discharge and continuity of care needs Qualifications

Associate Degree in Nursing or RN Diploma required Bachelor\'s Degree in Nursing preferred 1+ years in acute care or inpatient rehab required Experience in case management preferred Current license as a registered professional nurse in the state(s) of practice and/or has an active compact license, in accordance with law and regulation required Certified Rehab Registered Nurse (CRRN) preferred Certification in Case Management preferred BLS required Benefits
