Home health physical therapist to join Residential Home Health and Hospice, part of Graham Healthcare Group, delivering patient care in a team-based home health environment. Responsibilities
Collaborate to provide management and delivery of patient care plans in the home health setting. Requirements
Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required Licensed Physical Therapist issued by the state in which you will be working Minimum of one year healthcare experience Current driver’s license and ability to spend ~20% of your day driving to/from patient locations A commitment to consistently meet critical deadlines for charting The skills needed to self-manage your time and schedule Demonstrated experience with tablets, mobile phones and EMR software Compensation & Benefits
