Physical Therapist (PT)
for a
full-time
position providing
home health visits
in the Maryland area, specifically in
West Baltimore County . This role covers Reisterstown, Owings Mills, Randallstown, and Pikesville. Applicants must have at least one year of clinical experience as a licensed Physical Therapist. The role involves working as part of a multidisciplinary team to deliver rehabilitative care and skilled nursing, enabling clients to remain at home. Conduct home visits within designated territories. Perform diagnostic tests such as mobility assessments, strength, balance, and safety evaluations. Implement wound care interventions using evidence-based dressings and topical agents. Develop and revise individualized care plans, including manual exercises, gait training, and balance interventions. Assess and update care plans regularly, collaborating with nursing staff. Educate clients and families on rehabilitative activities to promote health and independence. Document client interactions and care activities accurately using electronic tablets. Qualifications:
Maryland Physical Therapist license. Minimum of one year of recent PT experience. Graduation from an accredited program by the American Physical Therapy Association or equivalent. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem-solving skills. Proficiency in computer skills; experience with EMR systems preferred.
