Learn More About Employee Benefits Job Summary And Qualifications The clinical pharmacist shall practice the profession of pharmacy with the utmost code of ethics demonstrating the organization’s mission and values. The clinical pharmacist is responsible for the safe, effective, evidence‑based, and cost‑effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of their work. They serve as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. Adherence to all regulatory and medication safety guidelines is at the forefront of all actions and decisions made within the practice of the clinical pharmacist.

What you will do in this role:

Prepare and dispense medication orders per physician request according to established policies, procedures, and protocols.

Verify Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness and intervene when orders are unclear, potentially unsafe, or inappropriate.

Interpret medication orders (verbal and written) and transcribe to computerized patient medication profiles accurately following facility practices and procedures for documentation in the patient’s medical record.

Trouble‑shoot/clarify Centralized Order Entry (COE) orders.

Prepare and dispense medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines.

Evaluate Total Parenteral Nutrition (TPN) for appropriateness based on facility policies and guidelines or ASPEN guidelines or recommendations as appropriate.

Issue controlled substances to patient care areas and maintain records as required by law and follow corporate policies and procedures related to dispensing and monitoring of controlled substances.

Ensure safe, appropriate, evidence‑based, and cost‑effective medication therapies for patients according to established policies, procedures, and protocols.

Monitor medication therapy regimens for contraindications, drug‑drug interactions, drug‑food interactions, drug‑laboratory test interactions, allergies, appropriateness of drug and dose, indications and therapeutic duplications.

Develop, assist, implement, and execute all drug therapy management protocols including, but not limited to; pharmacokinetics, anticoagulation dosing, IV to PO, pain management, antimicrobial stewardship, and pharmacy‑directed dosing.

Review/interpret culture and susceptibility data for antibiotic appropriateness and recommend changes as needed for drug‑bug mismatch, de‑escalation and duration of therapy.

Read, extract, and interpret information in patient charts accurately.

Detect and report suspected adverse drug reactions and medication errors accurately and in a timely manner.

Share good catches and clinical outcomes to pharmacy leadership to elevate pharmacist interventions and services.

What qualifications you will need:

Education & Experience: B.S. or Pharm D. degree in Pharmacy Required.

Graduate of an accredited PGY‑1 residency program or 2 years inpatient hospital pharmacy experience Preferred.

Maintains current pharmacist licensure Required.

Pharmacist Board Certification Preferred.
