With a variety of accessible and convenient mobile programs, the client provides an easy and convenient way to access healthcare services, regardless of location or mobility. The client provides comprehensive healthcare services through mobile and telehealth programs, focusing on accessibility for patients with chronic conditions or mobility issues. They offer services including mobile primary care, palliative care, wound care, transitional care post-hospitalization, weight management, behavioral health telehealth, and home-based physical, speech, and occupational therapies. Their programs are designed to deliver personalized care, improve patient outcomes, and reduce hospital readmissions, with 24/7 support and coordination with home health agencies and primary care providers. To assist with expansion and growth,
we are now hiring multiple full-time W2 or 1099 Mobile Primary Care Nurse Practitioner (ANP, FNP)
covering
Phoenix Central and/or East Valley Essential Functions Diagnose and treat patients in the comfort of their own homes and provide services for common chronic diseases via in-person and telehealth visits Prescribe all necessary medications and treatments Perform comprehensive physical examinations of patients Develop and implement patient management policies and procedures Facilitate referrals to other healthcare professionals and medical facilities Communicate with collaborating physicians or specialists regarding patient care Qualifications: FNP or ANP board-certified AZ License must be current and in good standing Initial experience working as a Primary Care Nurse Practitioner Must have current and active DEA License Ability to work efficiently and independently Ability to thrive in a fast-paced environment Excellent written and verbal communication skills Being bilingual is a bonus' Good to know: Onboarding and trainings will be
