Overview
With the program planning team and consumer, develop, implement and monitor the residential habilitation plan and IPOP. Implement the Individualized Service Plan (ISP) as developed for each consumer by his/her family advocate and Service Coordinator. Ensure that the consumer’s medical needs are met by coordinating medical appointments, arranging transportation, communicating with nursing staff, the Service Coordinator, family advocate, and residential staff, and scheduling follow-up or referrals as needed. Coordinate transportation as needed for consumers. Responsibilities
Responsible for the hiring, orientation and ongoing training, supervision and evaluation of all direct care staff, including the site supervisor assistant Work with the community services nurse to supervise and evaluate the performance of all medication administration certified staff, including developing and monitoring corrective action plans as needed, in conformance with agency policy and procedures Schedule direct care staff to ensure adequate staffing to meet consumer needs With the site supervisor assistant, ensure a supervisor is available 7 days a week during key program hours (weeknights, holidays and weekends) Become certified in medication administration and maintain certification on an annual basis Provide direct care services, as needed Complete monthly paperwork accurately (e.g., attendance records, petty cash, med fund, gas fund, monthly receipts, personal allowance records) Ensure residential habilitation services are documented in accordance with Agency, OMRDD and Medicaid requirements Ensure agency vehicles are maintained and required transportation paperwork is complete, per policy Prepare and submit management reports, as requested/needed Responsible for ensuring site is managed within budget Responsible for upkeep of the residence, including requisitioning maintenance for repairs to certified homes Participate in on-call rotation Ensure there are adequate and appropriate recreational activities in the community scheduled for consumers year round Oversee that the nutritional needs of the consumers are met and that balanced and healthy menus are prepared or available for consumers who prepare their own meals Participates in corporate compliance and quality assurance activities as required Participates in quality improvement and corporate compliance activities as needed Demonstrates adherence to the DSP Code of Ethics Support the Brand Platform of the Agency by incorporating the brand personalities Competent, Dedicated, Professional, Caring and Innovative throughout daily work Other related duties as assigned Non Essential Functions
Supports best practices and changing service delivery in response to changing consumer and/or staff needs Strives for consumer and employee satisfaction At all times, meets or exceeds professional standards as defined by position, agency and regulatory bodies Participates in quality improvement and corporate compliance activity as needed Participates in agency meetings as needed to further enhance service role and identify needs Professionally conducts self in promoting, contributing to, and implementing the mission, philosophy and policies of the Agency Consumer, employee, family, and community interactions are to be respectful and demonstrate professionalism, courtesy and consideration Agency, consumer, and employee confidentiality are maintained at all times Serves on agency committees as assigned/requested Qualifications/Experience Required
Minimum of a high school diploma or equivalent with 5 years\' experience working with developmentally disabled adults, OR Associate\'s Degree in Human Services or related field and at least two years experience with developmentally disabled adults, OR Bachelor\'s Degree in Human Services or related field with at least one year experience providing service to people with developmental disabilities is preferred. LPN or RN license may be substituted for Associate\'s Degree. A minimum of one year supervisory experience, with excellent supervision skills, is required Valid NYS driver\'s license with a safe driving record per agency policy: use of personal vehicle is required Additional Information
