Customer Success Manager - Care Partners
Join ChronicCareIQ as a Customer Success Manager (CSM) to lead partner success, retention, and revenue growth for our Care Partners (CP) customer base.
Position Overview We are seeking a CSM to serve as the trusted advisor and primary point of contact for CPs, driving partner success, identifying growth opportunities, and consistently expanding revenue.
Key Responsibilities
Partner Relationship & Success: Own the CP portfolio, lead QBRs, executive check‑ins, and in‑person meetings; develop tailored business plans aligned to revenue expansion.
Revenue Growth & Expansion: Drive upsell and cross‑sell of CCIQ solutions across care management programs; own revenue growth targets; equip CPs with sales enablement tools and pro‑forma ROI models.
Enablement & Marketing Support: Deliver sales workshops, webinars, and best‑practice sessions; partner with Marketing on co‑branded initiatives; support partner‑led pipeline development.
Recognition & Retention: Launch and manage the Premier Care Partners Program; track partner satisfaction (NPS) and mitigate churn.
Key Metrics (KPIs)
Revenue growth and upsell from Care Partners
Expansion of programs across the partner base
Sub‑account and patient volume growth
Partner churn below target threshold
Partner satisfaction (NPS above target score)
Number of Premier CPs established by year‑end
Engagement in events, workshops, and enablement programs
Qualifications
5–7 years in Customer Success, Partner Management, or Strategic Account Management, ideally in healthcare SaaS or EHR environments.
Proven track record of driving revenue growth, upsell, and expansion in a partner or channel model.
Experience balancing commercial accountability with relationship management.
Strong communication and executive presence with the ability to influence senior stakeholders.
Skilled at sales enablement, workshops, and co‑selling with partners.
Analytical and data‑driven; comfortable owning a revenue number and KPIs.
Bonus: Understanding of care management programs and provider workflows.
