MDS COORDINATOR
Join AHMC HealthCare as a
MDS Coordinator
at Seton Medical Center and Seton Coastside. The MDS Coordinator nurse will lead the MDS Program, assess and monitor patients’ health in accordance with medical laws, regulations, and guidelines.
Collect patient data, organize information into reports for process improvement projects, and communicate with healthcare teams to create applicable patient care plans.
Responsibilities
Facilitate all aspects of the MDS program, collaborating with physicians, nurses, and leadership.
Assess evidence‑based guidelines to determine care provision and initiate performance improvement activities.
Maintain MDS Coordinator records, including CMS submissions of assessments and CMS journals.
Create and maintain policies and procedures for MDS Coordinator practices.
Adhere to legal and recommended treatment guidelines pertinent to the MDS Coordinator and patient safety.
Skills and Knowledge
Self‑motivated and independent worker.
Comfortable teaching, guiding, and mentoring peers.
Excellent verbal, written, and interpersonal communication skills.
Qualifications and Certifications
Registered Nurse – Required.
State Licensure – CA – Required.
Minimum one year of experience in a healthcare setting – Required.
BLS – Required.
MDS certified – Required.
