The Personal Service Coordinators are responsible for coordinating the needs of each client and family in their caseload. Pay: $24.00-$26.00/hour Essential Duties/Responsibilities
Conducts intake, completes comprehensive evaluation of each client’s life situation, problems and needs, and with client’s participation and consent, prepares a detailed plan of care Responsible for service coordination by assisting clients in implementing their plan of care by facilitating access to and providing community resource linkage and managing the process to include monitoring effectiveness of support services including both client and provider participation Provides professional support to clients by maintaining regular contact to provide personal support, consultation, confrontation and intervention, as necessary, including guidance and instruction Maintains daily record of work activities completing daily service progress notes and billing forms per Company and government standards and in compliance with Short Doyle/Medi-Cal requirements related to case management Consistently meets monthly productivity requirement per hours established Develop and maintain professional working relationships with agencies and service providers. Maintain and ensures HIPAA, ethical standards and professional boundaries with all staff and clients Ability to work independently and make sound decisions without close supervision. Transport clients to accommodate community relationships. Use of personal car or vehicle is required May handle as assigned of On-Call Phone / After Hours duties Other duties as assigned or necessary to support your department/program, office and/or the company Recovery Requirements

Under direct supervision of the Lead Personal Service Coordinator and/or Program Supervisor Education And/or Experience
Bachelor's degree from an accredited university in human services or related field At least 2 years of experience working with adults with mental health issues or related field preferred Certificates, Licenses, Registrations
Proof of: valid California driver’s license and auto insurance, as well as proof of education are required. Knowledge, Skills And Experience
May be required to have knowledge of rehabilitation model Excellent documentation and organization skills, interpersonal skills and good written and verbal communication skills Intermediate level PC skills required Physical Requirements
Office Environment. Requires extensive sitting with periodic standing and walking. May be required to lift up to 20 pounds. Requires significant use of computer, tablet, phone and/or general office equipment. Needs adequate visual acuity, ability to grasp and handle objects. Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone. Will require off-site travel 20% - local and out of state. Will use personal car or vehicle as required for travel and/ or transportation. What We Offer
