Reports to the Concierge Coordinator Position Summary

As a Licensed Home Care Nurse, you’ll bring exceptional care and comfort directly to patients’ homes. You’ll assess health needs in the home setting, provide medications and treatments per provider orders, and help patients and families feel confident managing care. You’ll monitor progress, coordinate with other providers, and ensure visits support safety, independence, and dignity. Your compassion, attention to detail, and commitment to patient well-being will make a lasting difference. Essential Functions

Assess patient health status in the home environment Administer medications and treatments as prescribed in adherence to provider orders Monitor vital signs and symptoms to track progress or identify complications Educate patients and families on managing conditions, medications, and safety at home Coordinate care with physicians, therapists, and other providers Document care provided and update electronic health records (EHRs) accurately Ensure safety and infection control in the home setting Support patients’ independence and comfort in alignment with care goals Qualifications

Education: Graduation from an accredited nursing program (bachelor's or master’s degree preferred) Licensure: Active and unrestricted State of Michigan license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN); or Active and unrestricted Michigan-issued RN or NP license; Active and unrestricted Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) National Board Certification from one of the following organizations: American Academy of Nurse Practitioners or American Nurses Credentialing Center; or Active and unrestricted State of Michigan RN and NP licensure with applicable specialty certification Experience: Minimum of 2 years providing care as a nurse in applicable areas (preferred) Basic Life Support (BLS) Certification from American Heart Association or American Red Cross within 90 days of employment Additional Certifications: Preferred in areas such as Health Coaching, Case Management, Primary Care, Med-surg/ICU/ED, Women’s or Men’s Health, Sports Medicine Physical Requirements

Ability to provide hands-on patient care in the home; standing, walking, bending, reaching, kneeling, lifting and transferring patients as needed Must be able to lift or carry up to 50 pounds occasionally Operate medical equipment; use computer and clinical tools Visual and auditory acuity for patient assessment and communication Reliable transportation for occasional travel (employee not responsible for transporting a client) Changing conditions may affect physical requirements Seniority level
