Overview
Family Medicine Nurse Practitioner or Physician Assistant opportunity with Cityblock Health, Inc. in Greensboro, NC. As a Pod Provider, you will deliver high-quality, person-centered care with a strong focus on chronic disease management, behavioral health, acute care, and preventative services. You will work collaboratively within a multidisciplinary team to create and implement care plans that meet the individual needs of our members. Key Responsibilities
Provide direct care for patients with chronic conditions such as hypertension, diabetes, and kidney disease Evaluate and manage acute concerns across all age groups Support behavioral health care, including depression, anxiety, and substance use disorders Deliver preventative care, including sexual and reproductive health services Conduct initial clinical intake visits and develop ongoing care plans Perform timely follow-up after hospital discharges, ED visits, or other transitions of care Collaborate with interdisciplinary team members to ensure comprehensive and coordinated care Contribute to the development of clinical programs and continuous quality improvement initiatives Qualifications
Active and unrestricted NP or PA license in the state of North Carolina (required) Minimum of 3 years clinical experience, preferably in primary care or chronic disease management Strong clinical judgment and communication skills Commitment to delivering equitable, team-based care Why Join Us?
