RN Hospice Home Care role at EvergreenHealth. Description Wage Range: $45.92 - $82.96 per hour. Bonus: Up to $10,000 for those new to EvergreenHealth. The posted wage ranges reflect the minimum to maximum for the role; some levels may have different pay scales or additional premiums based on shift, certifications or degrees. Job offers are determined based on years of relevant experience, level of education, and internal equity. Job Summary Responsible for the total direct nursing care of all patients within established standards of care, hospice, hospital and unit policies, procedures, and guidelines. Primary Duties Provides direct patient care. Assesses the comprehensive nursing needs of an assigned group of patients (including home health patients). Coordinates the patient and family education process. Initiates patient start of care through the established admission process. Continuously plans, implements, and evaluates patient care, incorporating patient and family into the process. Collaborates with physician partners and other interdisciplinary team members to develop and implement the plan of care. Communicates all relevant information to the supervisor in a timely way; documents changes in patient condition to the team, DPOA, and physician. Delivers nursing care within applicable standards and policies; acts as a patient advocate. Assists other staff as time permits; recognizes when additional help is needed to effectively execute the nursing process. Promotes good public relations between patient, family, hospital, and community; practices within the RN scope of practice.
