Staff Pharmacist-PRN SMH-East role at Ochsner Health. This job maintains and ensures complete and appropriate drug therapy in a multidisciplinary pharmaceutical care environment. Prepares, dispenses and verifies medications ordered for an assigned group of patients, using safe, rational, and cost-effective therapeutic decision making. Assesses, evaluates and monitors patient drug therapy; provides drug information, patient education, and other clinical pharmacy services. Responsibilities
Evaluate all physician orders based on a clinical assessment of the patient, including allergies, age, renal function, concomitant drug therapy, lab values, dosing guidelines, best practice protocols, diagnosis, and physical clinical status. Ensure safe and accurate preparation and dispensing of medications, including sterile products and investigational drugs, with QA, medication occurrences, competency assessment, and documentation. Manage drug therapy to optimize patient outcomes through intervention documentation, ADR assessment, chart reviews, target drug reviews, patient education, and provision of drug information. Maintain professional and technical knowledge, licensures and certifications through continuing education. Manage the operations of the pharmacy in compliance with state and federal regulations. Adapt communication and service delivery to the patient population, including privacy respect, introduction methods, explanation of services, and obtaining permissions. Qualifications
Education: Required - Bachelor of Science in Pharmacy. Preferred - Doctor of Pharmacy (Pharm.D). Certifications: Required - Current Pharmacy license in the state of practice. Work Experience: Preferred - Doctor of Pharmacy (Pharm.D); Other: None explicitly required. Knowledge, Skills, and Abilities (KSAs)
Must have computer skills and dexterity for data entry and retrieval of patient information; proficient with Windows-style applications and keyboard. Effective verbal and written communication skills; ability to present information clearly and professionally to varying levels of individuals throughout the patient care process. Skills necessary to compound pharmaceuticals, operate technical equipment, and related tasks. Excellent interpersonal skills. Equal Opportunity
