Speech Language Pathologist/SLP

Job Description: Develop effective treatment plans and obtain approval for services from referring physicians. Treat patients per the physician treatment plan. Communicate with supervisor and other health team members regarding patient progress, problems, and plans. Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed. Participate in in-services training programs for other staff in the facility. Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per Speech and Language Pathologist regulations and payer requirements. Record treatment changes per policies and procedures. Instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services. Participate in discharge planning. Act as a clinical preceptor for CFY/RFP Speech-Language Pathologists and student programs. Comply with the State Practice Act and facility policies and procedures. Relate positively, effectively, and appropriately with patients/residents, families, staff, and colleagues. Required license or certification: Bachelor's degree in Speech-Language Pathology; Master's or Doctorate preferred. Proficient in computer skills such as inputting data into Casamba and PointClickCare. Our Mission: Committed to excellence in serving our community.
