Overview Recruiting for one ambulatory hematology-oncology clinical specialist pharmacist position to cover GI, breast, and multiple myeloma clinics.
Responsibilities
Provides expert-level consultation, guidance, and direction for appropriate hematology and oncology-affiliated medication use.
In conjunction with a team of heme-onc providers, delivers complete patient care through consultative services at the Outpatient Cancer Center Clinic and active participation in multidisciplinary patient care planning meetings.
Optimizes outcomes for heme-onc patients with evidence-based, patient-centered drug therapies; monitors for drug-related problems and aims for medication cost containment.
Supports regulatory requirements and local quality programs related to safe medication use and systems to promote medication stewardship and surveillance.
Provides pharmacokinetic and/or pharmacogenomic consultations as required.
Detects, monitors, documents, and reports adverse drug reactions and medication errors.
Monitors therapy for appropriateness of use, dose, regimen, route, and interactions; communicates patient information and treatment efficacy/safety to care teams.
Provides in-service education to the heme-onc team and clinic staff; counsels patients and caregivers on medications and adherence.
Leads and participates in interdisciplinary committees, protocol and treatment plan development, and education of pharmacists, nurses, physicians, and other providers.
Coordinates patient issues and therapy recommendations with pharmacy staff; supports research activities as requested.
Participates in specialty team meetings, including transitional care management, scientific research, and educational teaching conferences; stays current with changing protocols and pharmacy standards.
Qualifications Required Qualifications
Graduation from a Pharmacy program accredited by the Accreditation Council for Pharmacy Education.
Completion of a PGY2 residency in a specialty practice area and current, active board certification in that specialty (exception: current PGY2 candidates must obtain board certification within 12 months of hire).
OR PGY1 residency in a specialty practice area with a minimum of two years of post-licensure experience in specialty practice activities as defined by BPS and current, active board certification in the specialty.
OR Four years of post-licensure experience in the last seven years with at least 50% in specialty practice activities and current, active board certification in the specialty (no residency required).
Additional Qualifications
Prior experience in assigned area of specialty (e.g., Transplant, Infectious Disease, Hematology/Oncology) preferred.
Knowledge of Joint Commission, Federal and state regulations governing pharmacist practice in acute care.
Strong communication skills and ability to collaborate with patients and other practitioners.
Ability to apply professional pharmacy methods and techniques effectively.
License and Certification
Current license from the AZ State Board of Pharmacy.
Current, active board certification in area of specialty (exception: PGY2 candidates must obtain board certification within 12 months of hire).
Compensation & Benefits
Compensation: $130,499.20 - $195,811.20
Benefits Eligible: Yes
Schedule: Full Time
Hours/Pay Period: 80
Schedule Details: Monday - Friday, 0800-1700
